A Gala Closing Date Squad - Performance A

The closing date for this gala is 9 November 2018.

The date for the gala is  7, 8 & 9 December 2018.

Cost per swim is £10.00.

The following must be completed before handing in your entry.

  • A gala entry form must be completed
  • A medical consent form must be included
  • All entries MUST be in a sealed envelope with Swimmers Name – Gala & Amount Paid on the front.
  • Please only hand your gala entries to Michelle or Janice.  If anyone has any issues please contact compsec@cityofsunderlandasc.co.uk
  • PLEASE ADD THE TIMES FOR EACH SWIM YOU ARE ENTERING.
