For the Attention of City of Sunderland ASC parents & swimmers ONLY.

The actual closing date for these championships is Monday 1st July at midday. Entries must be in before this date in order to be processed.

Here are the details for the above meet, to be held on the 7th & 8th of November.

This meet is open to swimmers in AGD, Platinum, Gold and Silver squads.

MEDICAL-INFORMATION-AND-CONSENT-FORM

Price per swim is £5 which is inclusive of admin fee.

Closing date to hand entries in by is Friday the 25th of September 7pm

ONLY hand your completed entry forms, medical consent forms and correct payment to Jo or Amanda, to avoid your entries not been submitted.

If you haven’t got times on rankings please speak to your coach for your training times and put these on your entry form.

Entry forms, payment and medical consent forms should be put into a sealed envelope and hand it in to Jo Boxall, Amanda Greggs or Viki Birchall.

Entry forms will not be accepted after the 1st of April at 7pm, you can hand it in any time before this date.
